What to Bring:
A Picture I.D. – Other forms of identification may be accepted in accordance with Department of Justice regulations. Identification must be valid and current. If you are under the age of 18, you must also have a parent or guardian with you to sign a statement giving us permission to fingerprint you.

Out-Of-State Fingerprint Card Processing For Illinois
We can process your fingerprints for all Illinois professional requirements including nurse, physician, private security, PERC card, cannabis and more. Download and print out the forms in the packet below. Detailed instructions for filling out the Out-Of-State Fingerprint Application forms are included in the forms packet.
